MOM-n-PA  is an annual two-day free dental clinic for underserved Pennsylvanians in which dental treatment is provided at NO COST to individuals who cannot afford dental care. All services are provided free of charge by members of organized dentistry and assisted by a host of volunteers. Professional Liability Insurance
Please see the below information regarding obtaining PL coverage for dentists and physicians.

Dentists:
If you are a dentist who does not currently have professional liability insurance or only has coverage at your place of employment, you may be eligible to receive free professional liability insurance for the two-day event. Please complete and return this form. All dentists with active professional liability policies will need to rely on their current policy for coverage. Contact Megan Geider for more information.

Physicians:
Physicians, please complete this application and a signed no loss statement and also provide:

• The reason you are seeking coverage on this policy (uninsured? retired? etc.)
• Status with the entity – Independent Contractor, Employee or Volunteer

Rates depicted are for new and never practiced dentists, licensed within the last twelve months and are valid for $1M/$3M coverage amounts. Rates increase until a mature policy is reached.
